Leo Dean Forman, 82, of Mechanicsburg, Ohio passed away Wednesday, December 3, 2014 in Mercy McAuley Center, Urbana, Ohio.

He was born in Concord, KY on June 15, 1932 to Rosevelt and Goldie (Hilterbrandt) Forman. Dean was a life-long dairyman and he loved his cows. He also enjoyed walking horses and golf.

Dean is survived by his children, Vickie (Terry) Prather, Allen Forman (Bobbie Uttermark), Joe Forman, Beverly Forman, Creig (April) Forman, Charles (Tracy) Forman, James (Dale) Forman; grandchildren, Josh (Emily) Prather, Brad Forman, Annalise Forman, Kaitlyn and Sam Forman, Toby Kate Forman, Seth Forman, Deanna Prather; great grandchildren, Owen and Allen Prather; brother, Mike (Belinda) Forman; brother-in-law, Mike (Barb) Foster; and several nieces and nephews

Preceded in death by his parents; his wife Toby (Foster) Forman; and son, Bobby Forman.

A private service will be held at the family’s convenience. Arrangements are entrusted to SKILLMAN, MCDONALD & VERNON Funeral Home, Mechanicsburg, Ohio. In lieu of flowers, memorial contributions may be made to Mercy McAuley Center, 906 Scioto Street, Urbana, Ohio 43078. Condolences may be expressed to the family at www.vernonfh.com.
